Diabetes Education

The North York Family Health Team Diabetes Education Program is made up of a nurse and dietitian, both certified diabetes educators.

 We offer the following services for adults:

1.  Group Classes:  Pre-diabetes, Type 2 diabetes

2. Individual Classes:  Pre-diabetes, Type 2 diabetes, Glucometer Teaching, Post-gestational diabetes and Insulin Start/Titration
