Cast Insurance Medicals
These are requested by film production companies to perform independent medical examinations on their personnel. The examinations may be needed so the companies can obtain insurance for personnel, notably actors. These examinations are not generally solicited and are not considered doctor-patient relationships; they are third-party examinations requested and paid for by the production companies. The assessment may include a physical examination (done by DR Blankenstein) urinalysis, blood work with a drug screen, EKG, and an x-ray (done by lab and Diagnostic Imaging facility) The physician completing the cast medical exam should not be the examinee’s personal physician.
When booking and Cast Medicals, we will need the following information,
All patient demographics, Date of Birth, Address, Health Insurance information (if a resident of Ontario) and Email address.
Billing information (who will be covering the cost of the appointment), Fax/ Email.
Consent forms signed by the patient or guardian to release medical information direct to Insurance company.
All Insurance company forms and fax number/email address to send requested documents,
Can schedule Virtual or in-person appointments.
For efficiency , Patient MUST sign ALL forms prior to the appointment
Medical Issues
If the Cast Member is a resident of Ontario they should contact their family doctor first, If the patient is not an Ontario resident or their family doctor is unable to accommodate, we will need all patient demographics including Pharmacy information , OHIP Health card Number or P.O. number for billing purposed for non Candian residents.
Majority of Medical issues do NEED to be seen in person, however virtual appointment may be suitable at the doctor’s discretion. If a site visit is being requested, please give us as much notice and lead time as possible so the MD can make appropriate arrangements.
Fee Schedule
$175.00 CAD - Standard in Office Appointment
$175.00 CAD + HST - Cast Insurance Medical
$50.00 CAD per 15 minutes - Virtual appointment for medial concern by phone/video conference
Sacrifice office hours premium Mon-Friday = $50.00 CAD
After Hours Premium - Evenings Mon- Friday after 5pm = $70.00 CAD
- Weekends until 11 pm = $100.00 CAD
- Nights 11 pm to 9 am = $125.00 CAD
$50.00 CAD + HST - Prescription renewals without and assessment
$200.00 CAD + HST - Any official note/documentation required by production (if more complex, then hourly rate applied)
$475.00 CAD - Standard set visit = $175.00 CAD Appointment + $300.00 CAD Travel Fee
$625.00 CAD - Set visit (during COVID/health outbreaks ) = $175.00 CAD Appointment + $450.00 CAD Travel Fee & PPE
$30.00 CAD - Injection fee
Medical examinations of non-resident (non Canadian) performing artists
The Canadian Medical Protective Association (CMPA ) advises physicians to have the person being examined sign the CMPA's Governing Law and Jurisdiction Agreement that ensures if there are any disputes, claims, or legal actions , they will be brought in the jurisdiction where the physician practices. As such, we request that the production and insurance companies enter into an agreement requiring that any disputes, claims, or legal actions be brought in the jurisdiction where the physician practices, in accordance with the laws in that jurisdiction.
