PRESCRIPTION RENEWALS

As we do not accept telephone requests by the pharmacy or patient, renewals should be faxed to (416) 229-6283 by your pharmacy.

• Renewals (at the request of the patient or pharmacy) without an office visit are considered an ‘uninsured service’ and are not covered by OHIP.

• There is a $35.00 fee for each faxed request (*see ‘uninsured services).

• To avoid running out of your medication, please allow 10 days for your request to process.

Pharmacists in Ontario are authorized to bridge up to 4 weeks worth of chronic medications to ensure there is no gap

Bridge supply of medication means a minimum supply of medication dispensed to a patient, to ensure availability until the patient can access the medication from a usual source. (Community physician or specialist)

All Prescription renewal requests should be sent to the original prescriber.
